In order to keep both you and I safe during a treatment, my clinic must operate in accordance with COVID-secure guidelines from the UK Government as well as the British Acupuncture Council.

You must download and fill out the form above. Ideally then email it back to me once completed. The form only has to be completed on the first visit.

If for any reason you cannot fill out the form, please contact me more than 24 hours before your appointment to make alternative arrangements.

In order to reduce unnecessary time spent in clinic and handling of pens and paper, we must have a telephone chat at least 24 hours before your treatemnt.

You will need to attend with your own surgical mask. I will wear a fluid-resistant mask throughout your treatment also.

Arrive on time for your appointment. The front door will be locked and I will be looking out for you. I will have hand sanitising gel for you to use. We will then go straight to the first-floor clinic room.

Bring your own clean towel or blanket in a sealed plastic bag, a bottle of water, and a pen.
